Your current state is saved as it is when you cast the spell. While this spell is in effect, you can move around normally, but must concentrate on the spell for at least a minute. Doing so will remove its requirement to be concentrated on, and lasts an hour. When the spell ends, your body and mind are brought back to the original place you casted the spell, recovering your HP, spell slots, and inventory to how it was before you cast the spell. This does prevent you from grabbing things and pulling it back, as the item will drop at where you vanish if it wasn't originally on your person at the start of the spell. You retain your experience, and if you gain enough to level up, you must finish a short or long rest to tally it.

Dying isn't an obstacle to the spell: if you die before the spell ends, you stay dead until it does, where you will still return as you were at casting. However, the trauma of being ripped from death is taxing, and it's a coin flip between being completely drained of spell slots, or drained of stamina, leaving you with 1 hit point.

Casting this spell as a ritual will allow you to target yourself and one other creature, with no concentration required, for the duration. However, there is a hard range of 1 mile.
